Supporter Looking For Ways To Help And Support My Best Friend

My best friend and I have known each other since we were 13 years old and both new students in our junior high school. Our relationship mostly resembles that of brother/sister, though we did date off and on in high school. We are now in our early 30's. His family life isn't very supportive, and his time working as a Fire Fighter/EMT and as a military contractor are the primary sources of his PTSD. I also suspect he has issues with pain killers, alcohol, and insomnia.

We've always been very close, but right now I don't know what I should do to support him and help him. I'm concerned that I may be enabling his problems. When he opens up and talks to me about his thoughts and feelings, sometimes he scares me.
