I had a similar problem about a year ago. I don't know if this will help but here's what I did:
Once I realized that I wasn't concentrating at all anymore and that I was becoming really frustrated, I decided to just set myself less difficult goals because achieving a little bit is better and more motivating than achieving nothing.
I tried things like “I will concentrate for 10 minutes and then I can take a break” or “I will concentrate until this lollipop is finished”.
When I noticed that I was starting to zone out again, I set myself a timer so that I could take a short break and then try again.
When I was working at home I would go for a short walk whenever I couldn't concentrate anymore. However, stopping after trying for 5 minutes was always a bad idea because I usually need some time before I can really concentrate.
Taking notes, writing down questions and drawing diagrams really helps me understand certain subjects.
I also liked to write down the things that I had managed to do during the day or tick of things from a “to do list” so that I wouldn't get too frustrated over the things I didn't do.
So those are the things that I remember that slowly got me back to being more concentrated. I missed a lot of important things though, so I had a tutor to help me catch up for a while.
Skipping school wasn't an option because that would have only made me feel worse.
