Coming off lamotrigine (Lamictal)

Does anyone have experience on coming off lamotrigine? I’ve cut my dose into half twice now (100mg -> 50mg and 50mg -> mg) and I’ve been absolutely batshit for a week or two the both times. Like, hypervigilance through the motherf*cking roof, panic and anxiety out of nowhere, everything feels insurmountable…

Anyone had experience like this? It’s driving me nuts.

The hypervigilance and panic spikes you're hitting are really common when your brain is adjusting to lower doses. Lamotrigine does a lot of stabilizing work in there, so when you drop it, especially when you drop it relatively quickly, your nervous system can go into overdrive for a bit. It's like your body's alarm system is suddenly turned all the way up and doesn't know how to dial back down yet.

A couple things I've heard from others going through this: some people found that going slower between cuts helped way more than they expected—like, giving their system a longer adjustment period between each drop. And a few have mentioned that the intense stuff (the panic, the hypervigilance) does tend to settle down after a week or two as your body recalibrates, even though I know that doesn't make it suck any less *right now*.

Have you been able to talk to your prescriber about how intense the drops have been? Sometimes they can help you figure out if you need to slow the taper down, or if there are things that might help you feel a bit more grounded during the adjustment periods.

How are you holding up otherwise? Do you have any grounding stuff that helps when the anxiety spikes hit?

Luckily, I do have some grounding tools available.
Do you have an exaple of a reasonable scheme for tapering off lamotrigine? I’m at 25mg per day now.
 
Go slow! I remember coming off lamotrogine was one that we had to do pretty slow - fairly certain they were monitoring for seizures (?), which I think can be a withdrawal side effect even if you’re not taking it to prevent seizures.

On the up side, the withdrawal effects were temporary.

Thanks, Sideways! I just hate it when my docs seem to be totally clueless about med withdrawal. My psych doc told me it’s okay to drop from 50mg to zero. Well, okay, I’d probably be in an asylum if I did that. Yeah, not gonna happen. Docs just don’t know about this stuff. Seriously, they don’t. And when you go online and search for ppl’s experiences they tell a whole different story.

I’d really love to be able to trust my pdocs but I simply can not.
 
I wish I could trust professionals but I learned that it is best to have some knowledge and not trust blindly. If possible, reduce the dose as little as possible and give it time to be safe.

If you’ve been on lamotrogine for quite a while, this is probably good motivation to switch who you go to for psychotropic prescriptions!😬

Withdrawal is something a lot of pdocs get flippant about. There’s limited research or information about withdrawals for a lot of medications (even ones known to have big withdrawal issues), and obviously drug companies have low motivation to ensure prescribers are across withdrawal issues the way they’re motivated to provide education about other issues.

Yep. It’s crazy how there are two completely different stories around coming off psych meds: the ”official” story of the medical/pharma industry and the story of the med using and not-using community. The first one says you can taper off quickly and, uh-oh, you were not ready to quit because your symptoms came back and you’re still depressed. The other one says you have to taper off really really slowly and if your symptoms return or worsen, that might be your brain adjusting, not necessarlly your depression making a return.

And yeah, we can only wonder which story the pharmaseutical industry prefers…

I planned a six-month detox from lamotrigine after being on it 5 years. I started at 100mg and reduced by 10 mg, waited until I didn't feel any withdrawal effects (headache, irritability, and insomnia), and then reduced again. The key is to reduce by small amounts at a time. I had previously attempted to get off it with negative results, and this time, instead of following my psychiatrist's recommendation (2 weeks) I took it really slow.
