I'm always cautious and alert, I try to stay away from people

1.5 years ago I had a very violent fight, I also beat the person in front of me excessively, after I woke up he made threats to me saying I will die etc. I didn't feel anything for the first few months, I just thought about it, then these thoughts made me constantly be on the alert and stay away from crowded and noisy environments, I was afraid that I would be killed at any moment, the environment was not very good, social media was effective in the development of this situation, I decided to go to a doctor and was told PTSD, I started taking medication but it didn't work, it triggered my eye pressure and I stopped, give me a suggestion, am I really sick or are the threats real, I can't tell the difference, someone help me, It's like someone and a few hitmen are after me.
 
So the mind would numb the memories and feelings of the initial event until your mind somehow decides you're ready to handle it so it makes sense you feel that way. For single events instead of compound trauma there isn't anything to compare it to from previous experience so it would hit like a load of bricks and cause all sorts of effects, incl. the paranoia.

It can also be the focus of your life and just waste time ruminating.

Now since it was a fight, there is a chance he said that out of anger in the moment and didn't mean it. Was whatever caused the fight resolved then and there or is it really necessary to hire hitman and stuff cause it would cost a lot of time and money so if there isn't anything (objects or information) you know they want then it's highly likely they're not spending resources on this. Have you had any actual signs that you're being tracked, severe anxiety doesn't count since it can mimick fear very well. I mean cold hard evidence. They're still people and can't hide everything.

Sounds like it's most likely over if they didn't do anything for so long. It's normal to get triggered by similar content even if you were only threatened without it happening. Maybe don't view those content if it's distressing.
 
Even if the threat passes, it left damage, that's a fact, I'm not a normal person, I can't get back to normal right now, maybe the traumas I experienced before were triggered, because I didn't live with a normal family, maybe there are genetic factors, I don't know, because it's not normal on my mother's side, I think I'll start medication and therapy, I've never told anyone before, (except the doctor) it felt good, thank you 🙏
