I am horrible to people and dont even know i am doing it. It takes me months to realise that i have been arrogant or selfish. It doesnt even dawn on me what ive done.
i have exploded on everyone i know and im isolated as a result.
I am praying that cbt works can anyone tell me hoe effective it is?

CBT and DBT can both be immensely effective, and offer great skills that can be then used not just in handling relationships better, but also to get more out of psychotherapy in due course.

If the issue is PTSD, CBT (especially trauma-focused CBT) can be all some people require. If it’s complex PTSD, often DBT is more helpful, and psychotherapy and/or EMDR might be useful as well.

In addition to what sideways said, medication might help you to stay cool, calm and collected and help you be more amicable and friendly to folks. It's not that we are bad people, it’s just that we feel so bad that it spills out into the open.
