I was being treated by someone who had trained extensively in dissociative disorders. He had diagnosed me with major depressive disorder initially, along with DDNOS, based primarily on clinical observation and history. I was completely unable to stay present during our sessions and, after we talked about what happened during those times when I "disappeared" - basically nothing, I just went blank, and this was something I did all the time - he determined I was dealing with a form of dissociation. I had a history of trauma, and also symptoms of depersonalization and derealization.
After I'd been seeing him for awhile, a couple of my insiders (alters) presented themselves in sessions. I was completely unaware of that - in fact, I called him later in the week, asking about the sessions I thought I had missed.
This happened over a few months.
So essentially, he used a combination of an extensive history, clinical observation, and my own reporting of symptoms.
