Yes & triggers can be subtle, but fuel panic & many other emotions quickly.

Funny, the only thing I recall being terrified of as a child was being left (when someone said they'd come back, but not if they just were not there), & being lost without resources (& not knowing which direction to walk). I actually cried at those times as a child, albeit privately usually.

I guess the focus becomes is this reaction (posted by the OP) the 'norm' consistently?

Yes, pinpointing the time it started happening in the whole scheme of things. What changed at that time. Funny, a GPS is working for the OP but for me GPS's send me around the bend. They confuse me even more. OP - sounds like you guys are fantastic - having someone there every morning and evening to help orient her? That is awesome! What about a service dog?

My favorite rule of thumb for dementia v. forgetfulness (esp. with aging because that's the first time most people start to lose acuity, but also other 'normal' neurological things)

If you forget where your keys are? That's forgetfulness (aging, sleep dep, etc. normal).
If you forget what keys are for? That's dementia.

((ETA: Not to which lock. We're talking do I drink them, throw them at trees in order to fly, wear them as a hat. Honestly not knowing what keys are and what they do.))
