IMO I don't think you lose the sense of pain but your threshold is put way up. Look at those martial artists, you can see them grimace, they feel it but not to the levels of a normal person.
My back surgeon asked me what pain level I was feeling when they were assessing me for the VA, I asked him which level, the civilian level or the military level as I believe you are conditioned for that too. Your are trained to push through the pain barriers which is actually bad. For instance, if you have lower back pain and you are carrying 100 pound on your back, then the right thing to do would be to stop, but that would not get the job done would it? See where I am going.
To me, pain meds just don't work. Reminds me of a song. Who sang it. 'The drugs don't work', 'They make you hurt', 'But I know I'll see your face again'.
