I don't really have trouble creating images in my head, just keeping them there... I think in pictures a lot, but the images will snap in and out of my mind so quickly that they don't make much of an impact. At least not when I want them to.. Even a flash of a bad image can induce a sudden fearful response that may lead to panic. When I want to hold on to a good image to create the false, good memory that is meant to desensitize myself to my fears, it won't last long enough to create the desired emotional response.
It's been a while since I posted this, and I have found the note cards and my recorded "guide" helpful. The note cards make it feel a bit like a choose your own adventure and prompt me to put more thought into the details my rushed images would leave out. The recorded guide helps keep my mind focused on a specific portion of the image so that I don't hurry it along.
I don't know if it will help you to try the same techniques, but so far it's been working for me! :)
