I used to be able to accomplish far more each day before the meltdown and going into therapy. I have learned to adapt and some days are better than others, but if other people are seeing good progress in you, well you are doing awesome so toss your lists and make new ones on a day to day basis is my suggestion. If you cannot afford to do this then do your best, but do not be hard on yourself for the days you do not accomplish so much okay? I think the wheelchair analogy is a good one for you and me too. We have all of our scars on the inside. I think you are doing awesome.
