I had a psychiatrist who sat behind her laptop the entire first session - directly looked at me maybe twice - all the while asking very probing questions. While that wasn't the only reason I didn't go back (she also issued an ultimatum regarding sharing information with my primary care physician), it certainly contributed to it.
My prior therapist didn't typically take notes, although I did see him eyeing his notepad once and told him to go ahead and write whatever he was thinking down :). My current therapist doesn't take notes during session, but does refer to them at times.
Personally, I would prefer they don't take notes during session, I find it distracting.
