Sometimes I wish there was a "sh*t my PTSD partner says" video because I'm sure "don't let me hold you back," or "do what you need to do," or "it is what it is," and more would all be included. Lol. I hate to make light of it because I know you're hurting. I just thought a little joke might help.
Seriously, I know it doesn't feel good and I hope you are taking care of yourself during this time. What I will say is let him contact you. I'd keep it lighter than "I may be moving 3 hours away." Just stuff like "good morning," "hope you're having a good day," or "I'm thinking of you, hope you're okay," are all things that don't require his response so he can just receive it and then feel welcome to contact you when he's ready.
If he doesn't, then be happy knowing you did all you could and you can find someone who's ready to be with you the way you want to be with him.
