Tracking Flashbacks?

Has anyone ever had a therapist ask them to "track" (or record, log, etc) their flashbacks? I should have asked my therapist for more clarification or for an example of what she wanted, but thought on Friday when she mentioned it that I could figure it out... or thought I'd be able to google it and find a worksheet or something, but no luck yet. She wants me to do this so that we could look at how often I am having them, when I am having them (days of the week or time of day) and look for triggers and other patterns, or signs that one might be coming on. So next week she wants me to bring in the log with me, she mentioned recording the date and time, and said something about how I felt before/after, but I'm not sure if she meant how I felt emotionally, or physically, or both, I'm not so good at the recording feelings part though. And I am having a hard time writing down any of the info without knowing exactly how to do it correctly, or having any idea how to format it, so was just curious if maybe someone here had an example that might be helpful?
 
That is interesting. I haven't had to do it. But it sounds like it could be of much use. I might start doing it for myself.
 
Hubby tracks his noting date,time and duration (appx) along with any obvious stressors/triggers. It has helped a little in the fact that it highlighted the times of day he was most vulnerable and some triggers we would of missed otherwise.
