Schedule A Disability Form (u.s. Gov Employment)

I'm graduating from school in about 10 months and a lot of the jobs in my area are in government. I've never done a government job application before but I hear that in the U.S. if you have a disability you're supposed to use a schedule A form. Anyone have experience with this? I'm just wondering what to expect since this is all new to me. Thanks.
 
I worked for the government and didn't have to do that form. When they asked on the paperwork about history I just said PTSD Hope that helps
 
I work for the government, and they just gave me a form asking if I had any of a list of disabilities. I could say yes, no, or not specify either way. PTSD was on there along with diabetes and other chronic and sometimes debilitating health problems.

I have since spoken to my superiors about PTSD in more detail, explaining which areas of my work it is more likely to affect and explaining that there may come a day when I take a sick day strictly for mental health.

No special forms, though?
