I do think that after 4 years, a new start with a new doctor is actually a really good thing. This guy sounds like a jerk, and like most of all, he isn't helping you reach your goals.
Don't see this as a reflection on you, but on him and his skill set. You are growing up and ready for a doc that's going to treat you like the bright and courageous young adult that you are, and respect you and collaborate with you on how to get better.
