True doctor's visits are much worse, albeit hopefully rarer than therapy. About $250 per appointment, plus $400-800 each for any immunizations or lab work. Rx's out of pocket run between $50-$1000 for a one month supply. Average ER visit is about 5k-10k. Average prenatal & birth 10k-15k. It's part of why we get a little crazy out here about insurance (having it, and whether or not it accepted by our doctors and therapists).
Community mental health programs are almost exclusively reserved for court ordered therapy for violent offenders, and are generally the bottom of the barrel anyway (new students, unaccredited, 3 week certification holders, etc. in a huge department overseen by 1 accredited director), and may be 1-3 appointments (with different people) per year for about 15minutes. Community medical simply doesn't exist. Generally each state has at least 1 hospital, sometimes more while some states not at all, required to take people regardless of their ability to pay... But only for life saving procedures, and you're still responsible for the bill. Things are a lot better with Obamacare. Before law change it would have been $1600 a month to buy insurance for my son & myself. Which was more than I made each month. We used to have over 40million people without insurance. I think it was only (hah, "only") 11million at present. But they're still pretty horrifying to most other 1st world countries. Yep, we've got amazing docs and top rate facilities with no wait times... If and only if, you can afford to pay.
There
are work-arounds for the truly desperate or determined. But it takes a lot of luck and hard work and time in order to learn to work outside of the system of cash-in-hand. Often, too, a lot of travel.
